  • Birth and marriage certificates, if available
  • Blue badge and bus pass, if applicable
  • Means of payment for copies (£11 each)

  • Full name at time of death & any other names used
  • Date & place of birth
  • Occupation & retirement status
  • Maiden surname (for married woman/widow) + spouse details
  • Current home address
  • Informant's full name, address & relationship

  • Marital status
  • Length of stay in hospital/hospice
  • Age (under 75 or not)
  • Previous occupation & industry
  • NHS number (if available)

Death Occurred at Home

 

  • Contact local GP
  • Call police if violent/unexpected death (don't touch anything)
  • Contact mosque/funeral director for guidance
  • Register death at local registrar

Death Occurred in Hospital/Hospice

 

  • Nurse/doctor informs next of kin
  • Identification of the deceased
  • Collecting personal possessions
  • Contact funeral director for further steps
  • Post mortem may be required

Burial in the UK

 

  • Receive death certificate & green burial certificate
  • Send certificate to funeral director (fax/email) before collection
  • Washing arrangements (family can attend)
